KOSCIUSKO COUNTY, Ind. -- The Kosciusko County Sheriff's Office reports deputies responded to S.R. 15 North near Waubee Road in Van Buren Township for a two-vehicle crash around 11:31 a.m., Thursday.

First responders arrived to find a Chevy Silverado, driven by 68-year-old Dale E. Wert of Goshen, and a Hyundai Accent, driven by 19-year-old Sarah Mischke of Milford, had crash head-on.

The cause of the crash is currently unknown.

Wert was found to be critically injured before being taken to Lutheran Fort Wayne where he later died from his injuries.

Mischke was pronounced dead at the scene by the Kosciusko County Coroner.

The crash remains under investigation by the Kosciusko County FACT Team.
