MIDDLEBURY, Ind. -- The Elkhart County Health Department is hosting an MMR vaccine clinic on Oct. 20 at First Mennonite Church in Middlebury for anyone ages 1 and up.

The cost is $15 per shot, billed to insurance if eligible.

Health officials say you must bring immunization records with you and if applicable, insurance cards for billing. Uninsured patients will pay out of pocket.

You must also bring your photo ID. Children must also be accompanied by a parent or an adult 18 or older with written permission from the parent.

Guardians must present legal paperwork.

No appointment is needed as it is a walk-in clinic.

The clinic runs from 2 to 5 p.m. in Fellowship Hall.
