Milton Village dementia care center opens in South Bend

SOUTH BEND, Ind. -- The Center for Hospice Care and REAL Services, Inc. announced the opening of the Milton Village. An immersive, community-integrated day center, the program cares for those afflicted with dementia.

It is the first of its kind in the nation to be modeled off of the Vivium Group’s Hogeweyk Care Concept.

Supported by several local individuals and organizations, the CHC and REAL Services began the project in 2019 in collaboration with the Hospice Foundation (HF).

Now completed, the project successfully converted the former CHC building into a modern dementia village based off of the Vivium Group’s community village in Weesp, Netherlands.

Two organizations are housed within the Milton Village:

• Milton Adult Day Services, operated by the CHC, provides day-to-day care for dementia patients
• Alzheimer’s & Dementia Services of Northern Indiana, operated by REAL Services, Inc., supports family members of Alzheimer's and dementia patients while advocating through community outreach and education.

 The new facility is located at 111 Sunnybrook Court in South Bend.
